About PACE

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care.

One Team One Mission

Position Summary :

The West PACE Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home, working in close coordination with PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function. The Physical Therapist collaborates with the Occupational Therapist to efficiently operate West PACE rehabilitation department and, also, works effectively with colleagues and stakeholders to promote the West PACE values, team culture and mission.

Essential Functions :

  • Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.
  • Deliver and document Physical Therapy interventions in accordance with physician orders and / or as agreed upon in the participants' care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.
  • Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs(FMP).
  • Provide training and instructions to staff, participants and caregivers / families in the use and care of therapeutic equipment.
  • Order, track and manage maintenance for equipment.
  • Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants' condition or functional status.
  • Work with the other members of the IDT to support smooth care transitions between settings (e.g. hospitals, skilled nursing facilities, home,)
  • In coordination with the Marketing Team, support enrollment of prospective participants into the program.
  • Participate in end-of-life care coordination and support.
  • Complete all required documentation in a timely and accurate manner.
  • Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families.
  • Follow West PACE policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
  • Other duties as assigned.
  • Education and Certification :

  • Valid Physical Therapy license in the state of California, or proof of license.
  • Master's degree in Physical Therapy;
  • Master's degree in a Physical Therapy related field in addition to a bachelor's degree in Physical Therapy; or
  • Current Basic Life Support (BLS) / First Aid Certification required
  • Current driver's license and proof of auto insurance required
  • Skills and Experience :

  • Minimum of two years Physical Therapy experience in a clinical setting with a frail or elderly population.
  • Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.
  • Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
  • Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
  • Experience and competency working with people from diverse backgrounds.

  • Working Conditions and Physical Demands :

    The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this role. In compliance with ADA requirements, reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

  • Work Environment : Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care. The work setting is in an Adult Day Health Center, primary care clinic and participant home environment with moderate noise levels and controlled temperatures.
  • Physical Requirements - Requires corrected vision and hearing to normal range. Must be able to communicate with all staff, vendors, and clients. Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment. Requires lifting / carrying up to 35 lbs. and pushing or pulling 50 lbs. using appropriate body mechanics and equipment, with reasonable accommodation if needed.
  • Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.
  • Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.
  • Environmental Conditions - May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. May be subjected to unpleasant odors.
  • Medical Clearance :

    Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
