Job descriptionHours / Day : 8 Hours / Week : 40 Shift : Day, 8a–5p (No on-call, weekends, or OT) Travel : Required within Colorado Springs area Active CO RN or Multi-State License Reliable transportation and car insurance required Preferred : HCHB experience (Hospice Care) Conduct comprehensive assessments for high-risk members Collaborate with PCPs to implement individualized care plans Identify and reassess cost-effective, appropriate levels of care Coordinate non-medical support (e.g., housing, transportation) Manage care transitions with physicians, social workers, and discharge planners Maintain detailed records of clinical, functional, and fiscal outcomes Promote health and prevent illness through proactive interventions Act as patient advocate, ensuring privacy and confidentiality Associate’s or Bachelor’s Degree in Nursing or related field Current, unrestricted RN license Case Management certification Strong analytical skills for complex medical decision-making
