Shift : MondayFriday days with potential weekend rotation and on call

Job Description :

Sunbelt Staffing is seeking a skilled Hospice Home Health RN for a contract role in Wytheville, VA. This assignment offers the chance to provide compassionate hospice care to patients in the comfort of their homes while supporting families with dignity and professionalism. Candidates should have strong hospice experience and be comfortable working independently with rotating weekend and on call responsibilities.

Key Responsibilities :

Deliver hospice nursing care to patients in home environments

Assess patient needs and provide symptom management

Collaborate with the interdisciplinary care team

Provide family education and emotional support

Complete timely documentation using Homecare Homebase

Qualifications :

Active RN license in Virginia

At least 2 years of overall RN experience

Minimum 1 year of hospice home health experience required

Current BLS certification

Experience with Homecare Homebase EMR

Must have reliable vehicle, valid drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
