Location : Onsite – Must live within 1 hour of 13501 Park Vista Blvd Ste 150, Fort Worth, TX 76177
Pay Rate : $60.00 per hour
Duration : 3-month contract (onsite required for entire duration)
Position Overview
We are seeking an experienced and patient-centered Clinical Pharmacist to join our team in Fort Worth, TX. This role is heavily focused on direct patient counseling , evidence-based pharmacotherapy review, and coordination of care. The ideal candidate is passionate about patient education, comfortable spending 7–8 hours per day on the phone , and committed to delivering exceptional service.
This position requires onsite work Monday–Friday and an active Texas pharmacist license in good standing .
Clinical Pharmacist Key Responsibilities
Clinical Responsibilities
Conduct comprehensive pharmacotherapy reviews, including drug utilization, counseling, adherence support, and interaction / side effect screening
Document clinical reviews clearly in the patient chart for follow-up nursing assessments
Provide counseling and education on new drug therapies following clinical management models
Complete required REMS training and adhere to program SOPs
Coordinate patient care with referring clinicians, addressing social, psychological, and physical needs
Serve as a liaison between patients, caregivers, clinicians, home health, and hospice providers
Provide subcutaneous injection teaching and coordinate nursing support when needed
Participate in on-call coverage as directed by the Pharmacist-in-Charge
Accurately process and verify prescriptions in accordance with pharmacy regulations
Manage patient therapy discontinuations and update corresponding records
Implement clinical management models for disease states and specialty products
Handle primary intake for new referrals
Deliver “outrageous customer service” in all patient and clinician interactions
Build strong, trust-based relationships with patients and providers
Perform intake and reporting of adverse events and product quality complaints per regulatory requirements
