Job Title : Pharmacist

Department / Location : Pharmacy / Chicago, IL

Shifts :

  • Rotating shifts, including weekends and holidays
  • Weekdays : A shift (8 am to 4 : 30 pm), P shift (11 am to 7 : 30 pm);
  • Weekend / Holidays : One shift (8 am to 4 : 30 pm)

Part-time (40 hours per pay period) + benefit eligible

Compensation : Pay is determined by various factors, including but not limited to job-specific requirements, individual experience, internal pay equity, and organizational needs / budget considerations, all in alignment with our RML's compensation philosophy. Additionally, you may be eligible for a discretionary annual incentive program based on both individual and organizational performance.

  • GENERAL SUMMARY :

    Staff pharmacist fills orders for parental and enteral medications, monitors patient drug therapies, and provides drug information in the institutional setting with general training in adult and geriatric pharmacy practice. Provides age and developmentally appropriate care to adult and geriatric patients. The pharmacist supervises and directs support personnel. Staff pharmacist reports to the Pharmacy Manager and Director of Pharmacy Services, in their absence a staff pharmacist is required to assume the essential responsibilities and perform the duties of the Pharmacy Manager and Director of Pharmacy Services.

    PRINCIPAL DUTIES AND RESPONSIBILITIES :

  • Supports the Mission and Vision of RML Specialty Hospital by demonstrating RML's Core Values of Service, Teamwork, Accountability, Integrity, Respect and Stewardship (STAIRS) in all actions. Promotes a positive patient / customer experience by exemplifying the principles of the Language of Caring in all interactions with patients, families and co-workers.
  • Adheres to the standard of care, standards of practice, policies and procedures of RML Specialty Hospital. Complies with the laws, regulations and standards of the various local, state and federal, and accreditation agencies, e.g. Illinois Pharmacy Practice Act, The Joint Commission, DEA, FDA, CMS. Complies the hospital and departmental Compliance Program
  • Supports the Mission and Vision of RML Specialty Hospital by demonstrating RML’s core values of “Service, Teamwork, Accountability, Integrity, Respect, and Stewardship” in all actions. Promotes a positive patient / customer experience by exemplifying the principles of the language of caring in all interactions with patients, families and coworkers.
  • Ensures the accuracy of the preparation and dispensing of medication orders per physician request according to established policies, procedures, and protocols.
  • Interprets, transcribes and verifies medication orders (verbal and electronic), screens for appropriateness and potential problems. Maintains accurate and complete patient medication profiles.
  • Contacts prescribers regarding unclear or problematic medication orders.
  • Supervises and ensures sterile medication preparation in accordance with USP Chapter 797
  • Supervises and ensures hazardous drug preparation in accordance with NIOSH guidelines and USP Chapter 800 standards.
  • Supervises and ensures the non-sterile medication compounding is in accordance with USP Chapter 795
  • Checks accuracy of drug cartfill, IVs, and automated drug dispensing machines.
  • Monitors the patient’s TPN requirements, calculates the TPN formulas, and submits them to outside vendors within the allotted time frame.
  • Controls use of investigational drugs as approved by the Institutional Review Board.
  • Maintains logs, records, and other required documentation accurately.
  • Responsible for the handling of all medications including procurement, storage, prescribing, preparation, dispensing, disposal, and monitoring in the pharmacy and patient care areas.
  • Responsible for Controlled Substances.
  • Issues controlled substances to patient care areas.

  • Reviews nursing and pharmacy reports and documentation for accuracy.
  • Investigates, resolves, and reports problems to the Pharmacy Manager and Director of Pharmacy Services.
  • Contributes to the prevention of drug diversion.
  • Provides clinical expertise :
  • Consultation and clarification to practitioners. Assures accurate, adequate, and timely drug information is supplied to the patients, family, and the professional staff.

  • Documents clinical activities and interventions accurately and completely.
  • Medication dosing and monitoring : e.g. kinetics, renal dosing, enoxaparin
  • Ensures medication reconciliation is performed accurately and timely for patients upon admission and at discharge. Conducts discharge education, which includes preparing discharge prescriptions, drug information and documentation.
  • Participates in the orientation, education, and training of the pharmacy, medical, nursing, and other staff.
  • Investigates and reports on adverse drug reactions, medication errors, and drug recalls.
  • Monitors drug therapy regimens for contraindications, therapeutic duplications, drug-drug interactions, drug-food interactions, drug-disease interactions, allergies, and appropriateness of drug and dose. Ability to meet the pharmaceutical care needs of the adult and geriatric age groups.
  • Adheres to financial compliance; appropriately processing of patient charges and credits.
  • Contributes to the management of the department budget.
  • Identifies and provides justification for capital equipment needs.

  • Maintains inventory levels to ensure cost efficiency.
  • Effectively communicates cost-effective medication use with the medical staff.
  • Sustains the drug formulary by minimizing non-formulary procurements, utilizing therapeutic substitution, and promoting rational drug therapy selection.
  • Supervises the work of the pharmacy technicians, students, interns, volunteers, and oversees the daily routine workflow. Contributes to the performance appraisal of pharmacy personnel. Intervenes as necessary to correct staff behaviors and re-direct work.
  • Performs the duties of a pharmacy technician as needed.
  • Remains current with developments in the field of pharmacy and ensures the enhancement of personal knowledge and skills by participating in seminars, in-services, reading pertinent literature, etc. Reads medical, pharmaceutical and other literature to determine its applicability to pharmacy and the organization.
  • Meets all annual and ongoing employee requirements. Completes all competence / skill requirements.
  • Complies with hospital and HIPAA policies regarding confidentiality of the patient’s medical information, verbally, on-line, and in print. Complies with all hospital and HIPAA policies related to computer use and system security. Intervenes to prevent inappropriate communications among staff, especially in public places.
  • Actively participates in staff meetings, safety huddle, education activities (excluding the requirements of state law), committee or task forces, and attends meetings as may be assigned.
  • Demonstrates efforts to improve patient satisfaction, lower costs, and improve quality.
  • Adheres to departmental standards :
  • Punctual, dependable, report to work as scheduled.
  • Maintain a neat, professional, well-groomed appearance.
  • Always wear hospital ID badge.
  • Provide professional, courteous, cooperative, and timely service to patients, visitors, and staff.
  • Report any abuse, neglect, or other patient concerns to the Director of Pharmacy immediately.
  • Work cooperatively and actively support all departmental staff.
  • Organize and prioritize work assignments.
  • Work passed scheduled departure time when required.
  • Respond to conflict discreetly and appropriately.
  • Demonstrate effective oral and written communication skills.
  • Answer the telephone in a courteous manner.
  • Inform other pharmacy staff, Director, and Manager of real or potential problems in a timely manner.
  • Assume the essential responsibilities and performs the duties of the Pharmacy Manager and Director of Pharmacy Services in their absence.
  • Perform other duties as assigned by the Pharmacy Manager or Director of Pharmacy Services.

    Knowledge of computers and software applications

    Knowledge & Experience

    Graduate of an ACPE-accredited School of Pharmacy with a B.S. of Pharmacy degree.

    Knowledge of regulations and laws related to the practice of pharmacy

    Pharm.D. preferred

    Minimum of one-year experience in a hospital pharmacy is preferred.

    License & Certifications

    Current licensure for pharmacists by the Illinois Department of Professional Regulation.

    Working Conditions

    Occasional travel / work between hospital campuses may be required.

    Repetitive use of hands, e.g., preparing IV admixtures, use of computer keyboard.

    May require lifting and carrying light loads (up to 30 lbs.) including boxes, equipment, unit-dose cassettes, and IV solutions.

    Stooping or kneeling to pick items up from the floor, e.g., to remove and replace items on lower shelves.

    Sitting, walking, or standing for long periods of time, 4-8 hours is often necessary.

    Must be able to physically operate the equipment used for the job.

    OCCUPATIONAL HAZARDS

    Potential for exposure to infectious patients and materials.

    Potential for exposure to hazardous and toxic substances, which may include chemotherapy, cytotoxic drugs, and cleaning solutions.

    Potential for sticks or cuts by needles and other sharp items.

    Potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.
