Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
Inpatient Pharmacist Position in the Charleston, WV
250 bed munity hospital, part of larger areahealth system.
Variable day / afternoon shifts.
Essential Duties :
- Reviews, processes, and dispenses physician medication orders and prescriptions
- Prepares and supervises the preparation IV admixtures and chemotherapy
- Provides therapeutic drug monitoring consultation, including determination of appropriate loading dose, maintenance dose, and predicted blood levels, based on patient specific parameters and disease process.
- Maintains routine workflow, integrates non-routine work and relays information as necessary to the oning shift.
- Consistently monitors and follows-up with unresolved medication order issues
- Monitors lab orders, department messages and follows up as needed
- Properly documents clinical interventions in the EHR system using provided guidelines.
- Knowledge and experience with Pyxis and other electronic Med stations
- Monitors and supervises Technicians activity, checks all medications prior to being filled into Med Stations
- Maintains all emergency drugs during shift
Qualifications, Skills, and Experience :
BS in Pharmacy or PharmD1 year of experience in an inpatient pharmacyActive WV Pharmacist License.p : starting at $70.00 / hour total
