• medical case management\n
• screening\n
• education\n
• counseling\n
• medication management\n
• networking with community health care providers\n
• participating in a rotating on-call schedule, primarily providing remote phone consultations, ensuring accessibility without the need for on-site presence\n\nThis position earns a wage of $29.05 to $43.57 hourly.\n\nIn addition to competitive pay, we offer our employees the following benefits : \n
