Our team is supported by full-time, dedicated physicians and nurse practitioners who specialize in hospice and palliative medicine.\n\nLocation : Fargo, ND\n\nSchedule : Flexible scheduling with autonomy to tailor visits to patient needs (M-F)\n\nTeam Meetings : On-site once weekly\n\nDay-to-Day Responsibilities : \n

• 50 / 50 split between home visits and assisted living facility visits\n
• Expectation to see 4 8 patients per day depending on acuity and travel time\n
• Collaborate with interdisciplinary care teams\n
• Deliver exceptional, mission-aligned care to patients in a variety of home-based settings\n\nRequirements : \n
• Minimum of 3 years of NP / PA experience in primary care with a focus on geriatrics\n
• Strong preference for prior home-based care experience\n
• Current licensure in North Dakota and Minnesota (or ability to obtain)\n
• Active DEA license\n
• Valid driver s license and auto insurance\n
• Ability to travel regularly within the designated territory\n
• Experience in hospice and / or palliative care is highly preferred\n\nCompensation & Benefits : \n
