Registered Nurse – Home Health

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. As a key member of the care delivery team, you will play a vital role in supporting patients on their health journey—right where they feel most comfortable : at home.

Position Summary :

The Registered Nurse – Home Health provides personalized, high-quality nursing care to patients in their homes. This role allows for autonomy in your practice, offers flexible scheduling, and provides the opportunity to build meaningful relationships through 1 : 1 patient care. You’ll work closely with a supportive interdisciplinary team to coordinate and deliver the right care at the right time.

Key Responsibilities :

Perform initial and ongoing assessments of patients’ physical, psychological, social, and environmental needs.

Develop and implement individualized care plans in coordination with patients, families, and the interdisciplinary team.

Reassess patients regularly and adjust care plans as needed based on changes in condition or circumstances.

Communicate significant findings and changes in patient status to supervisors, physicians, and other care team members.

Maintain accurate clinical documentation in compliance with regulatory and organizational standards.

Uphold patient confidentiality and privacy in accordance with HIPAA and other regulations.

Collaborate with other clinicians to ensure coordinated, efficient, and high-quality care.

Qualifications : Current, unrestricted RN license in the state of practice

Minimum of 1 year of nursing experience in a clinical care setting

Valid driver’s license, reliable transportation, and active auto insurance

Willingness and ability to travel within assigned service area (up to 50%)

Associate’s or Bachelor’s Degree in Nursing (preferred)

Prior experience in Home Health or Hospice (preferred)

Familiarity with HomeCare HomeBase or similar EMR systems (required)

Familiarity with OASIS (required)

Will Support The Following Counties & Areas : Hampshire County- Amherst, Blandford, Chester, Chesterfield, Cummington, Easthampton, Goshen, Granville, Hadley, Hatfield, Huntington, Middlefield, Montgomery, Northampton, Pelham, Plainfield, Russell, Southampton, Southwick, Tolland, Westfield, Westhampton, Williamsburg, Worthington

Hampden County- Belchertown, Chicopee, Granby, Holyoke, Ludlow, South Hadley, Ware, Agawam, Brimfield, East Longmeadow, Hampden, Holland, Longmeadow, Monson, Palmer, Springfield, Wales, West Springfield, and Wilbraham.
