Job descriptionGraduate of an accredited LVN program. Possess a current California LVN license. Interest, demonstrated ability and one-year experience working with the elderly or frail population required. Ability to work independently and as part of multidisciplinary team. Cantonese-speaker preferred, not required. POSITION SUMMARY
Under the direction and supervision of the Home Care Manager, the Licensed Vocational Nurse provides direct nursing services to participants.
DUTIES / RESPONSIBILITIES :
- Provides skilled nursing care to all participants including but not limited to administration and monitoring medications, checking vital signs and monitoring participant's changes.
- Supervises, trains, and monitors geriatric aides. Teaches participants and family members regarding participant needs at home.
- Communicates effectively with all team members, participants, and families. Participates in interdisciplinary team planning and problem-solving.
- Other related duties as assigned.
