Position Summary : The Healthy Relationship Program offers flexible, community-based services tailored to the unique needs of families, offering intensive family engagement, evidence-based education, and mentoring.
Youth Coaches are vital members of the Healthy Relationship Team, working directly with youth to reduce the risk of out-of-home placements. They provide one-on-one mentorship, focusing on building the youth's strengths, fostering attachments and relationships to school, community, and family, and creating achievable goals. Coaches help youth develop independent life skills, connect them to mental health services, substance abuse treatment, employment opportunities, and pro-social activities, while fostering supportive networks. Familiar with the communities they serve, Youth Coaches understand how Social Determinants of Health impact quality-of-life outcomes and risks, ensuring their support is both relevant and effective.
