What You Bring

Licensure :

Licensed as an Advanced Practice Registered Nurse (APRN) or Physician in Connecticut.

Certification :

Board Certified as a Family Nurse Practitioner (FNP) or Family Practice Physician.

Experience :

At least one year of post-licensure primary care experience or three years of other post-licensure practice experience.

DEA Registration :

Active DEA Controlled Substance Registration.
