Associate Patient Care Coordinator
OPTUM OBGYN of Baldwin has an immediate opening for a friendly, patient focused and detail oriented Associate Patient Care Coordinator to join our team.
This position requires a 37.5-hour work week, with specific hours determined by the hiring manager upon hire. The anticipated schedule is :
Monday : 9 : 00 AM - 8 : 30 PM (8-hour shift within this range, to be determined by the hiring manager)
Tuesday : 9 : 00 AM - 5 : 30 PM
Wednesday & Thursday : 10 : 00 AM - 6 : 30 PM
Friday : 8 : 00 AM - 1 : 00 PM
Location : 650 Merrick Road, Baldwin, NY 11510
Primary Responsibilities :
- Obtain accurate and updated patient information, such as name, address, and insurance information
- Perform insurance verification on the date of service
- Obtain patient signatures for required documents
- Confirms and schedule appointments
- Follow the Cash Security policy and procedures according to Optum Medical guidelines
- Perform referral documentation promptly
- Utilizes computer systems to enter access or verify patient data in real-time ensuring accuracy and completeness of information
- Performs certain follow-up services for patients in a prompt and courteous manner, such as scheduling specialist appointments
- Adhere to the standards identified via Sparq regarding Optum Employee Policies
- Work cohesively with fellow employees to achieve specific team goals
- Comfortable working in a high-paced environment
- Assure the continuity of care through scheduling and tracking systems
- Provide effective communication to patient / family team members and other health care professionals as evidenced by documentation, case conferences, communication notes, and evaluations
- Responsible for collecting data directly from patients and referring provider offices to confirm and create scheduled appointments for patient services prior to hospital discharge
- Respond to patient and caregivers' inquiries related to routine and sensitive topics always in a compassionate and respectful manner
- Generates, reviews and analyzes patient data reports and follows up on issues and inconsistencies as necessary
- Comply with administrative policies to ensure quality of care
- All employees are expected to keep abreast on current medical requirements relevant to their position, which includes maintaining patient confidentiality and abiding by all HIPAA and OSHA requirements
- Ability to be cross trained as a medical assistant
- Ability to cover other offices as needed
- Performs other duties as assigned

Required Qualifications :
High School Diploma / GED (or higher)1+ years of customer service or healthcare related experience1+ years of computer proficiency / experienceAbility to work in a flexible scheduleAbility to travel to other offices if needed for coveragePreferred Qualifications :
1+ years of experience working in medical front office position performing duties such as scheduling appointments, checking patients in / out, insurance verification, and maintaining medical recordsExperience working with an electronic health record (EPIC)Knowledge of medical termsBilingual in English & SpanishSoft Skills :

The hourly pay for this role will range from $16.00 to $27.69 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
