Position Summary

The Goals of Care Registered Nurse is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S / he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Goals of Care Registered Nurse serves as a consultant for partner organizations to complete Goals of Care conversations and documentation with the organization's members. S / he supports the frontline provider by devoting time to family meetings dedicated to the discussion of treatment goals. S / he helps facilitate the completion of advanced care planning documents while applying the rules and regulations of state and federal regulatory agencies and other certified agencies. S / he serves as a clinical and educational role model and mentor, provides support in collaboration with the primary care team, and ensures compliance with practice standards and codes.

Position Specific Responsibilities

  • Performs Goals of Care conversations with members and / or families.
  • Educates patient and family to enhance understanding of disease processes and plan of care, including discussion of treatment options to ensure informed decision making.
  • Provides emotional support and counseling to patients and their families to improve transitions, adjustment to illness and adherence to treatment plan.
  • Empowers patients to take control of their health care by encouraging appropriate goal-setting and discussions about advance care planning / completion of advance directives.
  • Partners with healthcare team to manage transitions of care to hospice or palliative care when appropriate.
  • Assists patients and families in identification of goals of treatment and ongoing plans of care and communicates these goals to the healthcare team.
  • Helps triage new consults to prioritize case load.
  • Prepares and maintains accurate patient records, charts, and documents.
  • Performs other duties as assigned.

Education And / or Experience

  • Associate or Bachelor's degree in Nursing required.
  • Minimum of one (1) year experience in hospice and / or palliative care required.
  • Skills

  • Mathematical Skills : Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills : Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities : Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.
  • Certifications, Licenses, and Registrations

  • Active and unencumbered Registered Nurse license in the state(s) of employment.
  • Current CPR certification required.
  • Physical Demands and Work Environment : The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
