Community Health Navigator - Affordable Housing

As the largest nonprofit affordable housing provider in the nation, we build, preserve, and manage program-enriched housing across the country.

The Community Health Navigator will develop strategies to link residents with health care and care coordination services. Develop relationships with the primary care clinics, medical providers, care coordinators, health plans and other health service providers to increase the health and wellness of residents and reduce unnecessary healthcare utilization. This is a grant-based position.

We encourage candidates with lived experience to apply. This position is a part-time, onsite position, approximately 24 hours per week.

Pay : $20.00-21.50 / hour, dependent on experience.

Local travel in Phoenix area required. Bilingual English / Spanish highly preferred.
