Experience & Qualifications

  • Years of Experience : 2 years of acute hospital Case Manager experience (care coordination and discharge planning in fast-paced environment)
  • Patient Ratio : 1 : 20
  • Charting System : Epic
  • Certifications : BLS
  • Hospital Experience :

    • Teaching Hospital – Preferred

  • Community Hospital – Preferred
  • LTAC – Preferred
  • Trauma Level I & II – Preferred
  • Work Settings : Acute Hospital, Ambulatory, Behavioral Health, Home Health, Hospice, Insurance Company, Long Term Acute Care / Rehab / Skilled Nursing
  • Special Requirements : Must have experience organizing family meetings, developing treatment plans, coordinating DME, SNF, and ambulance transport independently
