Position Summary
Under the general supervision of the LifeSpring Program Manager, provides intensive case management and independent living skills development to foster youth and probation youth transitioning into independent living. Assist youth in developing skills, obtaining employment, pursuing educational goals, and developing life plans that will support independence.
Services are provided in a client-centered, trauma-informed, and culturally sensitive manner that is consistent with Lifeline's philosophy and mission to build self-sufficiency. Possess knowledge and expertise in relationship to transitional age youth, specifically former foster youth, and the specific issues surrounding this population and demonstrate excellent engagement skills.
Provide skill development opportunities for youth including life skills training, meditation, economic literacy, transportation assistance, public benefits advocacy, financial assistance, and tenancy training. Provide supportive services to parenting youth including individualized housing, assistance securing childcare, education and support, maternity / paternity provisions, and specialized services for children.
Provides linkage to services within the community to support transition to independence as well as aftercare, including but not limited to, mental health services, educational (college or post-secondary training) and employment support, etc. Provide crisis management and safety planning to the youth in the program.
Case management will have a therapeutic component and evidenced based practices should be utilized. Participation in county collaborative meetings to identify service gaps, barriers, and successes in order to create best practices, policies, procedures, and protocols. Provide training and outreach and identify and seek partnerships with other agencies and organizations for collaboration.
