Must-Have : Proficiency with HomeCare HomeBase (HCHB)

Job Description :

• Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures, through the competent application of the nursing process.
• Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Perform home visits and attend periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Assigned between 5-10 patients per day, equating to roughly 6-8 points.
• Travel up to 75 miles from the home clinic is required, with mileage reimbursed according to the IRS standard rate.
• Required to be proficient in Oasis for RNs, PTs, and OTs.
• Must be willing to accept all patients assigned, regardless of distance from previous appointments.
• Documentation must be completed within 24 hours of the patient's appointment.
• No guaranteed hours. Must achieve a weekly productivity target of 30 points, equating to 40 hours per week.
• Flexibility to float to another nearby clinic to pick up patients if not receiving enough weekly patients to meet financial needs.
• Some positions may involve a Baylor Shift schedule : 4x10s, Friday-Monday, while most will be standard 5x8s, Monday-Friday.
• Proficiency in HomeCare HomeBase (HCHB) and used it within the last 6 months, or completion of a retraining course during compliance.
