Job Summary

We are seeking a dedicated and compassionate Licensed Physical Therapist to join our rehabilitation team. In this role, you will assess, plan, and implement individualized physical therapy treatment programs for assigned patients. You will work collaboratively within a multidisciplinary care team to help patients reach realistic and achievable recovery goals based on their physical capacity and tolerance.

Key Responsibilities

  • Perform initial assessments and evaluations to determine patients’ physical therapy needs.
  • Review medical records and clarify referrals to ensure appropriate treatment planning.
  • Select and apply appropriate assessment tools to guide individualized treatment strategies.
  • Develop and implement comprehensive, medically prescribed therapy plans.
  • Document patient progress and maintain accurate clinical records.
  • Educate patients and families on prescribed physical therapy programs and at-home care strategies.
  • Collaborate with patients, caregivers, and other healthcare providers to create and implement effective discharge plans.
  • Request additional treatment orders as necessary for unfamiliar diagnoses.

    • Qualifications

  • Bachelor’s Degree in Physical Therapy (required).
  • Active and valid New York State Physical Therapy License (required).
  • 0–1 year of experience as a Physical Therapist (new graduates welcome).
  • Strong communication and interpersonal skills.
  • Bilingual (English / Spanish) strongly preferred.
