Job Description : Travel LPN – Long Term Care Facility – Beaver Dam, WI

Position Title : Travel Licensed Practical Nurse (LPN)

Location : Beaver Dam, Wisconsin

Facility Type : Long-Term Care / Skilled Nursing Facility (LTC / SNF)

Shift : Days, 5x8 schedule (40 hours per week) with every other weekend rotation

Pay Range : Competitive weekly gross pay (based on assignment)

Responsibilities

Provide direct nursing care to residents in a long-term care / skilled nursing facility.

Administer medications and treatments as prescribed by physicians.

Monitor residents’ health status and report changes promptly to supervising RN or physician.

Assist residents with daily living activities while promoting independence and dignity.

Document care provided and maintain accurate medical records in compliance with facility and regulatory standards.

Collaborate with interdisciplinary teams to develop and implement individualized care plans.

Support infection control, safety, and quality improvement initiatives.

Provide compassionate care and maintain professional communication with residents, families, and staff.

Requirements

Current Licensed Practical Nurse (LPN) license in Wisconsin (or compact license).

Current BLS certification (American Heart Association).

Minimum of 1 year of experience in a long-term care or skilled nursing facility.

Strong clinical skills and ability to work independently in a fast-paced environment.

Excellent communication, organizational, and patient care skills.

Experience with electronic medical records (EMR) systems preferred.
