As a Telehealth Triage Coordinator, you'll be the essential link between patients and remote Infectious Diseases Physicians, facilitating real-time consultations in a hospital setting. Using a mobile telehealth cart, you'll guide and support each visit - positioning patients, assisting with physical assessments, and ensuring the physician can clearly visualize and interact with the patient. You'll also help triage patient needs and provide health information under the direct supervision of the physician.

Key Responsibilities :
Coordinate Telehealth Visits
Schedule patient appointments, ensuring alignment with physician availability.
Act as a liaison between local providers and remote Infectious Diseases Physicians.
Prepare for each visit by reviewing patient records and identifying clinical goals.
Verify patient consent for telehealth services.
Support Clinical Evaluations
Operate and manage the mobile telehealth cart during patient exams.
Perform physician-directed physical assessments using telehealth tools (e.g., camera, connected stethoscope).
Assist with wound care, patient positioning, and other hands-on support as directed.
Patient Communication & Education
Provide health information and anticipatory guidance under physician supervision.
Educate patients on infectious disease management and medications.
Schedule follow-up appointments as needed.
Documentation & Communication
Accurately document patient interactions and triage calls.
Communicate complex cases and verbal orders to physicians.
Coordinate with departments and pharmacies to address patient needs.
Administrative & Technical Support
Order labs and tests per standing orders; process prescription refills.
Maintain HIPAA compliance and uphold privacy standards.
Manage the telehealth cart, troubleshoot technical issues, and coordinate cleaning and maintenance.
Quality & Compliance
Reconcile visit documentation for billing accuracy.
Assist with data collection and medication reconciliations.
Demonstrate system-wide competencies and professional behavior.
Performs in accordance with system-wide competencies / behavior.
High School Diploma, GED required.
Successful completion of Licensed Practical Nurse course OR Medical Assistant
1 Year of experience working with patients in a medical setting preferred (office or hospital setting)Licensure, Certifications, and Clearances :
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
Licensed Practical Nurse (LPN) OR Medical Assistant
