Home Health Nursing Professional

Our Home Health Licensed Practical Nurses play a pivotal role in patient care management, working closely with interdisciplinary teams to assess, plan, implement and evaluate individualized care plans.

Key responsibilities include :

Conducting thorough assessments to identify patient needs

Developing and implementing effective care plans to address these needs

Providing high-quality patient care services

Maintaining accurate and up-to-date records of patient interactions and outcomes

Collaborating with healthcare professionals to ensure seamless care coordination

Demonstrating exceptional communication and interpersonal skills

The ideal candidate will possess a strong foundation in practical nursing principles, with a Graduate degree from an accredited or approved school of practical nursing. A current license to practice practical nursing in the State is also required, as well as CPR certification and at least one year of practical nursing experience. Home health experience is highly desirable.
