Summary :
A brand-new PACE (Program of All-Inclusive Care for the Elderly) center is seeking a dedicated Nurse Practitioner to join its interdisciplinary care team (IDT). This comprehensive healthcare model empowers seniors to safely age in place by providing coordinated medical, rehabilitative, and social services in one supportive environment.
This role is ideal for an NP who is passionate about geriatric care, excited about building and growing a new center, and confident functioning as a clinical leader within a collaborative, team-based model. The census will start low, offering the opportunity to help build workflows, establish processes, and shape the future of the center.
Day to Day :
- Serve as a core member of the interdisciplinary care team (IDT), collaborating with social workers, PT / OT, dietitians, and other providers
- Conduct comprehensive health histories, physical exams, and intake assessments for new participants
- Perform ongoing follow-up assessments based on participant needs
- Integrate the primary care treatment plan into the broader IDT-developed care plan
- Manage care for participants located in contracted facilities (nursing homes, hospitals, assisted living)
- Evaluate acute concerns, new symptoms, and changes in condition
- Facilitate referrals to outside medical specialists
- Provide preventive health maintenance, including immunizations and recommended screenings
- Prescribe, adjust, and monitor medications
- Order and interpret diagnostic exams such as labs or imaging
- Provide education on therapeutic diets and lifestyle recommendations
- Participate in infection control and surveillance activities in collaboration with the Medical Director
- Travel up to 50% between the PACE center, participant homes, and contracted care facilities (as census grows)
- Report directly to the Medical Director
- Support the launch of a brand-new PACE site—establishing workflows, processes, and team structure in the first several months
Qualifications : Must-Haves :
Graduate of an accredited school of NursingCurrent registration and license to practice as a Family, Adult, or Geriatric Nurse PractitionerAANP or ANCC Board CertificationValid DEA certificationBLS certification through AHA or Red CrossMinimum 2 years’ experience as an NP (Adult, Family, or Geriatric)At least 1 year working with frail or elderly patients , preferably in long-term care or primary careProven leadership skills with experience in program expansion, team growth, or new center start-upsStrong knowledge of value-based care, risk-based contracting, and Medicare / Medicaid regulatory frameworksNice-to-Haves :
Experience in long-term care, PACE programs, assisted living, or geriatric-focused settingsStrong collaboration and communication skillsComfort with autonomous decision-makingAdaptability and enthusiasm for building a program from the ground upPerks! :
