Job Description
Immediate opening for a licensed therapist dedicated to improving mental health outcomes.
Responsibilities include :
- Conducting psychosocial assessments and developing patient evaluations
- Engaging with family and significant others for education, support, and advocacy
- Coordinating with community social and mental health agencies
- Facilitating discharge planning and post-discharge follow-up to reduce re-hospitalization
- Leading daily patient group sessions
