The Care Management Registered Nurse plays a vital role in providing comprehensive patient care by assessing, planning, implementing, and evaluating tailored care plans in collaboration with healthcare teams. Your responsibilities will include : \n
- Continuously assessing the patient's physical and psychosocial condition throughout their care journey.\n
- Developing, communicating, and documenting a patient-centered plan of care that reflects individual problems and interventions, actively involving the patient, family, and healthcare team.\n
- Implementing and coordinating care plans in collaboration with the healthcare team.\n
- Evaluating the effectiveness of the care plan, the patient's response, and discharge readiness, with revisions and referrals as necessary.\n
- Providing education based on the patient and family's learning needs, documenting outcomes clearly.\n
- Functioning as authorized in roles such as psychiatric emergency nurse, triage nurse, charge nurse, or preceptor as assigned.\n
- Meeting performance expectations in customer service, teamwork, resource utilization, and professional development.\n
- Performing additional duties to ensure the smooth operation of the department / unit.\n
- Recognizing the scope of your responsibilities and maintaining competency in relevant clinical and technical skills as per established standards.\n\nQualifications\n\nEducation : Graduate of an accredited nursing school; BSN preferred. Proficient in electronic patient records and internet research for patient care.\n\nExperience : Previous emergency department experience preferred.\n\nLicensure : Valid Registered Nurse license in the State of Connecticut.\n\nSpecial Skills : Current BLS certification required upon hire; ACLS and PALS required within six months; CPI required within one year; TNCC and CEN preferred.\n\nYNHHS Requisition ID : 162297
