We are looking for an Addiction Specialist to join our Program for Assertive Community Treatment (PACT) , a highly integrated, forensic-focused outreach and 24 / 7 residential program that reduces hospitalizations and incarcerations while supporting individuals in recovery and wellness.
What You'll Do
As an Addiction Specialist, you will play a key role in supporting individuals on their recovery journey, providing clinical, therapeutic, and case management services to help them achieve stability and independence.
Key responsibilities include :
- Delivering community-based services within the PACT forensic model, including therapy, medication administration, ADLs support, and care coordination
- Conducting substance use evaluations, creating recovery plans, and providing crisis / risk assessments
- Facilitating dual-diagnosis recovery groups and linking individuals to AA, NA, detox, or medication-assisted treatment programs
- Offering outreach services in homes, hospitals, and community settings
- Serving as a primary case manager for an assigned caseload, ensuring individualized, coordinated care
- Consulting with families and community agencies to enhance support systems
- Maintaining program documentation, treatment plans, and progress notes in compliance with agency standards
- Providing safe and timely transportation for appointments, activities, and daily living needs
