Behavioral Health Care Planner

A company is looking for a Care Planner Behavioral Health.

Key Responsibilities

Provide support to behavioral health clinical teams to enhance member engagement and care delivery

Educate members about specialty programs and assist in locating services

Collect patient progress information and perform administrative duties related to care transitions and authorizations

Required Qualifications

2 years of Behavioral Health care experience

Previous Customer Service experience

Proficiency with computer skills, including navigating multiple systems

Associates Degree or equivalent combination of education and experience
