POSITION SUMMARY :

The Stop Now and Plan (SNAP) Case Manager understands, implements, and promotes SNAP® curriculum in the community. This position utilizes strength-based perspective to help youth and families strengthen and manage the quality of their lives by engaging the family, advocating on their behalf, initiating change agent activities, helping them access support in the community, teaching problem solving skills, modeling productive behaviors, and assisting in the successful completion of youth and family development milestones.

WHAT YOU WILL DO :

  • Completes all SNAP® and agency-required staff training
  • Conducts intake and discharge assessments (SSIS, SSIS 2, HIT, CBCL, TRF, EARL-20B, PAT)
  • Manages daily case responsibilities for SNAP® for Youth participants
  • Schedules intakes and follows up on referrals
  • Makes weekly check-in calls to monitor youth / family progress
  • Facilitates SNAP® for Youth groups according to the model
  • Prepares weekly group materials (handouts, manuals, etc.)
  • Assists SNAP® Coordinator with group organization
  • Participates in team pre- briefing / debriefing and weekly consultation calls
  • Prepares tech for group sessions (Raspberry Pi, tablets, projector, etc.)
  • Maintains and updates data systems (NETMIS, JJIS, OneDrive, SNAP Spreadsheet)
  • Scores and documents group evaluations within 3 business days
  • Maintains detailed electronic and paper case records
  • Provides follow-up calls and assessments at 30 and 60 days post-discharge
  • Identifies and monitors at-risk families; informs supervisor
  • Refers families to community resources and ensures care continuity
  • Provides case management : assessment, planning, linking, monitoring, advocacy
  • Builds strong relationships with families, team members, supervisors, and community partners
  • Treats all youth and families with respect and courtesy
  • Participates in staff development, team meetings, and client reviews
  • Creates a safe, welcoming, trauma-informed environment
  • Practices self-care to prevent burnout
  • Follows agency policies and procedures
  • Upholds agency’s mission and core values

Location : Polk County

Requirements :

Education & Experience :

A high school diploma or equivalent and a minimum of 3-5 years of human service experience is required or a bachelor’s degree in social work or a related field. Experience with structured groups and parent education groups (preferred); community / agency liaison experience (preferred).

Knowledge, Skills & Abilities (KSAs) :

  • Excellent verbal and written communication skills
  • Solid knowledge of the rules of grammar and punctuation
  • Ability to display professional in day-to-day activities, communication, feedback, teamwork, and ongoing working relationship with outside agencies and organizations
  • Understand various problem-solving techniques
  • Ability to compile and organize information
  • Ability to work cooperatively in a group / team setting
  • Ability to facilitate small / large groups and varied group dynamics that may arise
  • Ability to take guidance and direction from supervisors
  • Proficiency in Microsoft Word and Excel, Dropbox, and video-conferencing programs
  • Skilled in actively listening to concerns, providing a safe space for individuals to express themselves without judgment or blame
  • Physical Requirements : Ability to bend, lift, and carry 10 pounds.
