Description :

The position is for a Home Health RN focusing on various patient cases.

Candidates must have experience in care for conditions such as CHF, diabetes, drain care, urinary catheter management, ostomy care, infusion services, central line care, joint replacement, and palliative care.

The RN will be responsible for providing care to adult and geriatric patients.

The expected patient visit ratio is 8 visits per day, with a 1 : 1 ratio of nurse to patient.

Charting will be done using Homecare Homebase, employing SBAR, SOAP, and OASIS E documentation methods.

Skills required include peripheral IV start, HCHB, infusion management, wound care, and V.A.C. management.

This position requires an individual to have flexibility as patients may be seen across various home settings.

Previous experience in managing drains and urinary catheters is essential.

Candidates should have a strong understanding of home health Medicare regulations.

The RN should also possess effective communication and interpersonal skills to navigate patient interactions successfully.
