Job descriptionAssess clients needs for services and cooperatively develop an intergraded care plan and goals with the client and other providers. Conduct comprehensive client intake, psychosocial needs assessments to determine client needs and service eligibility. Develop client-centered service plans, assessments and re-assessments in collaboration with clients and their care team. Monitor client progress and adjust service plans as needed to support goal achievement. Assess and identify the clients' needs, formulate appropriate plans of service, monitor and document the progress of service plans and document all significant interactions with or on behalf of the clients to ensure the clear, effective delivery of service and the provision of high-quality social services that are in compliance with the PHS / NYC DOH, Ryan White Part A requirements, and agency policies and procedures. Connect clients to mental health counseling, substance use treatment, HIV primary care, housing assistance, food security programs, and other supportive services. Assist clients in navigating Medicaid, Ryan White programs, ADAP, HOPWA, and other public benefits. Collaborate with healthcare providers and social service agencies to ensure holistic and integrated care. Work collaboratively with team members to provide outreach and engagement to hard-to-reach clients. Advocate for clients' rights and ensure they receive appropriate, stigma-free services. Address barriers to care such as homelessness, mental health stigma, discrimination, and financial instability, transportation etc. Accompany clients to key appointments and provide supportive counseling to enhance Work with the client to identify barriers to goal attainment and help resolve those barriers, i.e., keeping scheduled appointments with appropriate service providers, medication regimen for adherence while promoting good health. Provide advocacy and referral services to ensure clients receive appropriate benefits, entitlements and services. Provide short-term crisis intervention and de-escalation support. Educate clients about HIV treatment adherence, harm reduction, self-care, and mental health wellness. Facilitate peer support groups and workshops on behavioral health topics. Facilitate groups, as designated, to address wellness issues, enhance life skills, and assist clients in achieving self-sufficiency. Maintain accurate and timely case notes in Electronic Health Records (EHR) and other required databases. Ensure compliance with HIPAA, funder requirements, and agency policies. Participate in quality assurance and performance improvement initiatives. Document each encounter with each client via accurate progress notes. Attend and participate in Quality Improvement and Performance Improvement teams and tasks. Participate in program meetings, trainings, and client case conferences. Ensure adherence to client confidentiality procedures as governed by HIPAA and HCC confidentiality policies and procedures. Attends required meetings, conferences, supervision, trainings, and professional development. Associate degree in Social Work, Counseling, Human Services, Psychology, Public Health, or a related field required. Bachelors degree preferred. Bachelors degree in Social Work or Counseling preferred. At least 1 year of experience in case management, HIV / AIDS services, and / or preventive services. Experience facilitating groups. Strong cultural competency in working with diverse populations, including PLWHA, BIPOC, LGBTQ+, and immigrant communities. Knowledge of harm reduction, trauma-informed care, and motivational interviewing. Ability to work independently and collaboratively in a fast-paced environment. Strong organizational, communication, and problem-solving skills. Strong interpersonal and excellent verbal and written communication skills. Organizational and problem-solving abilities to assist clients with complex needs. Effective client engagement and needs assessment skills. Experience on NYC health and human services systems, processes, and services. Competency in Microsoft applications including Word, Excel, and Outlook. Language Proficiency (English / Haitian Creole, French and / or Spanish,) preferred. Must be able to pass a background check Passion for HCCs mission.

Case Manager
PURPOSE OF POSITION
The Case Manager is responsible for providing comprehensive case management services to individuals living with HIV / AIDS (PLWHA) who are engaged in supportive counseling and behavioral health services. This position ensures that clients receive coordinated, person-centered care that addresses their mental health, medical, social, and economic needs.
The Case Manager works closely with clinical and support staff to assess client needs, develop service plans, coordinate care, and provide advocacy and linkage to essential services such as housing, financial assistance, substance use treatment, and healthcare.
