Serving clients in their homes and community settings

What Youll Do :

• Provide hands-on support and companionship to clients as they transition between services or care locations.
• Assist with daily living activities and individualized care needs.
• Work closely with our care team to ensure smooth transitions and continuity of care.
• Travel to different client homes as assigned.
Position Details :
• Full-Time
• Must be FLEXIBLE with hours schedules vary based on client needs.
• Support clients one-on-one in their homes (no hospital or rehab assignments).

• Dependable, caring, and adaptable team member
• Valid drivers license and reliable transportation
• Previous caregiving experience preferred
