We go beyond traditional care by offering holistic health services and free transportation for patients, ensuring access and continuity of care. The Role

• Patient Load : 18 patients / day; max panel size of 450 patients
• Support : Dedicated team including your own MA and scheduler.
• Patient Population : Seniors with multiple chronic conditions, managed through a value-based care model .
• Additional perks : door-to-door patient transportation, in-house specialists (Cardiology, Podiatry, etc.), and holistic health services (Acupuncture, Tai Chi, Yoga, Nutrition).
• Compensation : $260K $265K base + annual performance bonus (up to $20K in year one, increases with partnership)
• Partnership : No buy-in required. Eligible within months; 65% of physicians achieve partner status within a year. Partners typically earn $300K+.
