University Home Care is seeking a compassionate Medical Social Worker to join our team in Livonia, MI. As a Medical Social Worker for Hospice, you will play a crucial role in providing emotional support and counseling to patients and their families during difficult times.

Responsibilities :

• Conduct psychosocial assessments of patients and families
• Develop care plans and provide support services
• Collaborate with interdisciplinary teams to ensure holistic care
• Provide counseling and education to patients and families
• Advocate for patients' rights and access to resources

Requirements :

Previous experience in hospice care preferred at (LEAST 1 YEAR)
