WeCare Centers at Heritage Care Center, located at 5701 Phillips Ave Pittsburgh PA 15217 is actively seeking dedicated Weekend Baylor - LPNs to add to our growing team!

Full Time : Saturday & Sunday 7a - 3p / 3p - 11p

Our LPN's provide essential nursing care, including monitoring patient health, administering medications, assisting with daily activities, and collaborating with the healthcare team to ensure quality resident care.

Job Duties

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered       Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.
