Position Overview

As a Hospice Case Manager, you will coordinate and deliver high-quality care to patients in their homes or care facilities. You’ll work autonomously while collaborating with an interdisciplinary team to ensure holistic, patient-centered support.

Key Responsibilities

• Develop and manage individualized care plans.
• Coordinate medical, emotional, and spiritual support services.
• Monitor patient progress and adjust care as needed.
• Educate patients and families on hospice care and symptom management.
• Collaborate with physicians, nurses, social workers, and other professionals.
• Maintain accurate, timely documentation using EMR systems.
• Participate in team meetings and quality improvement initiatives.
