Required Certifications :

AHA BLS, Licensure required for the State of Wisconsin.

NPI # required.

If the candidate does not have one, they need to start the process of applying for a NPI # in order to start.

Required Education :

Master's Degree in Physical Therapy.

Minimum Experience :

Home Health experience, training working with the adult / geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis. Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.
