PRN- Physical Therapist

Grace Reliant Health Services is looking for an experienced PRN Physical Therapist to join our growing Home Health team. Our skilled nurses and therapists offer excellent in home services to patients throughout Southeast Missouri. The ideal candidate will provide skilled Physical Therapy services on an intermittent basis to patients in their homes in accordance with physician orders.

This position serves patients in the following counties : Cape Girardeau, Perry, and Scott Counties.

Responsibilities for this position may include but are not limited to :

• Provide initial and ongoing comprehensive assessments of patient's needs
• Establish a treatment plan in consultation with the physician
• Treat patients to relieve pain, develop or restore function and maintain maximum performance
• Teach, supervise, and counsel the family and patient in the physical therapy program
• Record and report to the physician the patient's reaction to treatment and / or any changes in the patient's condition.
• Prepare an assessment of the patient's condition and periodic evaluations and summaries
• Record all evaluation data, treatments and patient's responses on a timely basis
• Participate with staff, patients, and physicians in discharge planning activities and complete a Physical Therapy Discharge summary.
• Willing and able to drive throughout the Southeast Missouri region

Requirements :

Graduate of Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association

Licencsed or Registered by the State of Missouri

Professional Liability Insurance

1+ Years of Professional Experience

Valid Driver's License and Car Insurance
