Rate CAPS for submission :

Malpractice

– Must be included in rates.

Orientation

– 275 - half the hourly / callback rate kicks in after 8 hours. “Per hour of

orientation or training beyond 8 hours”

On Call Daily – 5100 - “Per 24 hours to include 4 hours gratis

24 Hour Holiday Call Rate - 7650 - “Per 24 hours to include 4 hours gratis

Call Back

– 550 – “Per hour of

patient contact hours after gratis hours are completed gratis prorate to MSA

terms.”

Holiday Call Back – 825 – “Per holiday hour of patient contact

hours after gratis hours are completed gratis prorate to MSA terms.”

Facility Location : CHI Memorial - Chattanooga TNSetting : Acute Care / Inpatients / Interventional Neurology Lab / ORDuties : Procedure / Op Notes and Post NotesDays per month : TBDRequired Procedures : Endovascular Neuro Procedures Thrombectomies AneurysmEMR : EpicSupport Staff : Inpatient Hospital = APP 24 / 7 IN Lab = RN and RT both trained in Endovascular Neuro cases OR = normal OR contingent RN Surg Tech etc
