BHN is currently seeking a compassionate and motivated Case Manager (BA Level) to join our School-Based Community Service Agency (CSA) team in Springfield, MA.

What You'll Do
As a Case Manager (BA Level) , you will provide mentorship and support to middle school students (grades 6-8) at Chestnut Middle School, Chestnut TAG, Impact Prep, and Springfield Legacy Academy. You will work closely with a Master's Level Case Manager to help youth and their families build skills, strengthen relationships, and connect with essential community resources.
Maintain a caseload of 15-20 youth as assigned by the programMeet weekly with youth as a mentor to support prosocial skill development and healthy decision-makingCollaborate with the Master's Level Case Manager on individualized service planning and risk / safety assessmentsSupport youth and families with crisis planning and reassessment as neededCo-facilitate psychoeducational groups focused on social skills and reducing risk of substance useProvide parent education and skill-building sessions at least five times per yearAssist youth with academic tutoring and school-based supportsPartner with schools, providers, and community resources to ensure coordinated careSupport youth and families outside of school hours when neededAttend IEP / 504 meetings and contribute to treatment planningMaintain accurate daily progress notes and complete documentation in a timely mannerWho You Are
Bachelor's Degree in Counseling, Psychology, Social Work, or a related human services field requiredExperience working directly with youth, families, or individuals with behavioral health or substance use needsStrong communication, mentoring, and interpersonal skillsKnowledge of child-serving systems and community resources preferredCultural competence and ability to work with diverse populationsFamiliarity with documentation, word processing, and email systemsValid driver's license and reliable transportation requiredWe Hire for Purpose!
