EndoNeuro@Home is a mobile senior and diabetes clinic providing compassionate, personalized care to patients in the comfort of their own homes.

We specialize in diabetes management , chronic disease care , nutrition , and patient education . Our mission is to empower patients through medical care, lifestyle guidance, and preventive health strategies.

Our providers travel to patients’ homes in Plano and surrounding cities , using a company-provided vehicle (with gas included) and medical tools and supplies to deliver high-quality, in-home healthcare.

Position Overview

We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) passionate about senior care, diabetes management, and lifestyle education. The ideal candidate is compassionate, self-motivated, and eager to provide exceptional care outside of a traditional clinic setting.

Responsibilities

• Drive the company-provided vehicle to patients’ homes in Plano and nearby areas to deliver care
• Conduct new and follow-up office visits , annual wellness visits , preventive counseling , transitional care management , and special preventive services
• Prescribe, adjust, and refill medications according to clinical guidelines and patient needs
• Provide education and nutrition counseling focused on chronic disease management, diabetes, and preventive health
• Collaborate with physicians and the care team to create individualized treatment, nutrition, and education plans
• Document all patient encounters accurately and maintain compliance with clinical standards
• Build trusted, long-term relationships with patients and families
