Shift : Days / 8a-5 : 30p, 9x4, 36 hrs per week

Length : 13 weeks

On call rotation every 4 weeks for coverage when needed

Requirements :

WILL ACCEPT ILLINOIS LICENSE PENDING!

4x9s (8a-5 : 30p) and a half day (8a-12p)

Occupational Therapist : acute and outpatient OT.

Start date : ASAP

Hours : 40-hours M-F (can be flexible : 5-8s 8-430, with one late day 9-530, 4-10s 7-530, 4 9s (8-530) and a half day (8-12)

Years of experience REQ : 1

First-timers accepted : Yes

Certs REQ : BLS and IL OT state license

Is on-call REQ? on call rotation every 4 weeks for coverage when needed

Will clinician float within scope to meet facility needs? Yes; may float to Acute care and outpatient

Pending License accepted : No. License in hand

RTO Restrictions : Please request RTO up front if possible.

Guaranteed Hours : If confirmed to 3x12-hour shifts, SSM will guarantee a min. of 36 hours / week. If confirmed to 4x12-hour shifts, SSM will guarantee a min. of 48 hours / week. If confirmed to shift lengths less than 12-hours, SSM will guarantee a min. of 40 hours / week. If confirmed to shift lengths less than 12-hours, SSM will guarantee a min. of 40 hours / week. Please note : SSM may cancel (call-off) the following per 4-week period : Up to 8 hours for Candidates confirmed to 8-hour shifts. Up to 10 hours for Candidates confirmed to 10-hour shifts. Up to 12 hours for Candidates confirmed to 12-hour shifts.

Special Requirements : BLS and CPR certification required.
