What You'll Do :

1 : 1 Support : Provide case management on site and in a community-based setting to youth ages 14-24, supporting to navigate shelter and housing, educational systems, obtain employment, and re-engage with school

• Skills Groups : Facilitate life skills groups for at risk youth facing barriers to graduation
• Crisis Support : Support youth in navigating crisis and provide crisis interventions as appropriate.
• Data Compliance : Maintain required documentation, including case notes, case files and expenditures.
• This position is based at the Youth CNCT building at 41 SE 102nd Avenue and launches in community to meet with youth, returning to Youth CNCT or other New Avenues locations between meetings and at the end of the day
