As the RN Care Plan Coordinator , you’ll play a key role in guiding resident care through the creation, coordination, and monitoring of personalized care plans. You’ll collaborate with a multidisciplinary team, including physicians, nurses, therapists, and social workers, to ensure that each resident’s medical, emotional, and social needs are fully addressed.
Care Plan Coordinator (RN) Key Responsibilities :
Develop and implement individualized care plans tailored to each resident’s unique goals and health needs.
Collaborate closely with the care team to ensure care plans are carried out effectively and updated regularly.
Conduct comprehensive assessments and monitor progress, adjusting care plans as residents’ needs change.
Provide education and support to residents and families, helping them understand care plans and treatment goals.
Maintain accurate and timely documentation in compliance with state, federal, and facility standards.
Ensure care plan compliance with CMS, HIPAA, and North Carolina state regulations.
Participate in ongoing quality assurance and performance improvement initiatives.
Care Plan Coordinator Qualifications :
Current Registered Nurse (RN) license in the state of North Carolina (required).
Strong organizational and communication skills.
Experience in long-term care or skilled nursing preferred.
Ability to work collaboratively in a team environment.
