Job Details – Pediatric Home Care RN (Webster, NY) Location : Webster, NY 14580 (Rochester area – New York healthcare jobs) Setting : Pediatric Home Care (Certified Home Health Agency) Assignment Type : Travel RN Schedule : Monday – Friday, 8 : 00am – 4 : 30pm EST Hours per Week : 40 Shift Duration : 8 hours Assignment Length : 13 weeks (potential extension) Start Date : November 17, 2025 Estimated Weekly Salary : $2250 - $2310 Qualifications – Case Management RN Active New York State Registered Nurse (RN) license (required) Current BLS certification (accepted from any provider) Valid NYS driver’s license and current auto insurance Minimum 1 year of clinical nursing experience (pediatric nursing or home care experience preferred) Experience with OASIS assessments preferred Proficiency with Microsoft Office Suite and electronic medical record systems (EPIC or Netsmart a plus) Ability to travel throughout the Webster and Rochester service area year-round Strong interviewing, assessment, and communication skills Ability to work independently, exercise initiative, and demonstrate sound clinical judgment Key Responsibilities – Pediatric Home Care Case Manager RN Develop and implement individualized care plans in collaboration with patients, families, physicians, and the clinical team Complete OASIS assessments to accurately document clinical, functional, and social needs Ensure timely and accurate completion of clinical records for compliance with New York State and federal regulations Coordinate and refer patients to appropriate healthcare systems and community resources Communicate assessment outcomes and care plans to the healthcare team for effective follow-up Maintain productivity and adhere to professional home visit performance standards Demonstrate patience, courtesy, and flexibility while interacting with pediatric patients, families, and staff Resolve issues at the point of contact and maintain positive professional relationships Apply now to join our pediatric home care nursing team in Webster, NY, and take the next step in your travel RN career!
