Our client is seeking a compassionate Medical Social Worker to join their Hospice care team in Bay City, MI. This position provides the opportunity to make a meaningful difference in patients' lives through collaboration, care planning, and emotional support. The MSW will work closely with the care team to ensure each patient's needs are met with professionalism and empathy.

Responsibilities include providing and managing patient care plans, supporting patients and families in Bay City and surrounding areas, maintaining accurate documentation, and effectively managing time and travel requirements.

Schedule : Monday Friday, 8am 5pm, with rotating weekends.

Travel : Local travel within the assigned region required.

Required qualifications :

Master's degree in Social Work from an accredited program (CSWE-approved)

One to two years of post-graduate experience in a healthcare setting

Active MSW license in the State of Michigan

Valid driver's license, reliable transportation, and auto insurance

Experience using EMR software such as TruChart

Ability to meet charting deadlines and manage time independently
