Job Summary :
The Enrollment Case Manager is responsible for conducting in-home intake visits for Home and Community
- d Services (HCBS) waiver applicants within an assigned county and neighboring counties as needed. This role involves facilitating the eligibility determination process, assisting with application submission, and fostering relationships with community-based organizations and stakeholders. The position requires travel throughout the assigned county and the ability to work independently.

Responsibilities :
Travels throughout the assigned county and neighboring counties as dictated by business needs, working primarily in applicants' places of residence.Conducts intake visits for HCBS applicants and acts as a facilitator for the eligibility determination process.Presents and discusses the concept of HCBS programs, and advises applicants of their rights and responsibilities.Assists with application submission, including data entry of the Compass Application.Assists applicants with selecting an MCO, Life Provider or Support Coordinators that best meets their needs.Assists with developing relationships with community-based organizations, advocates and stakeholders.Fosters and maintains relationships with parties serving mutual consumers, including physicians, County Assistance Offices, providers, nursing homes and rehabilitation facilities.Facilitates the efficiency of the application process, ensuring any stalled cases receive extra attention and support.Attends regularly scheduled meetings.Reports on schedule availability at regular intervals.Occasionally lifts, carries, or otherwise moves items weighing up to 25 pounds.Performs other duties as assigned by management.Required Skills & Certifications :
