Assesses and plans treatment for direct patient care, oversees plan of care with PTA, responsible and accountable to know and understand all OZH policies, vision, mission, values and standards.

• Drives his / her own vehicle regularly to perform the essential functions of the job
• Current valid driver’s license
• Vehicle liability insurance coverage
• Clear or acceptable motor vehicle record
• Initial MVR and annual MVR
