JOB SUMMARY

The Case Worker I, reporting to the Program Director, is responsible for providing comprehensive case management services. This includes supporting participants in their daily activities, assisting them in setting and achieving short-term and long-term goals, and guiding them toward stable, long-term housing and safe exits from the shelter. This role also involves transporting participants to and from essential appointments to ensure they have access to the resources needed for their success and well-being.

ESSENTIAL JOB RESPONSIBILITIES

  • Complete participant service plans, assessments, HMIS data entry, and housing plans, while supporting them in achieving their identified goals. Provide residents with instruction, as related to specific housing needs.
  • Ensure participants understand and adhere to program rules by clearly explaining guidelines and providing them with a copy of the policies.
  • Maintain thorough and accurate participant files and documentation, including specialized incident reports, crisis interventions, medication changes, and any notable changes in behavior. Ensure all participant files and HMIS profiles are updated promptly and consistently to reflect the most current and accurate information.
  • Maintain positive relationships with all referral sources and community partners.
  • Collaborate with team members to effectively coordinate participant activities and foster social engagement. This includes leading and facilitating life skills groups to support their personal growth, skill development, and empowerment.
  • Serve as a resource for case staffing by collaborating with internal team members and participating in external conferencing to address complex decision-making challenges. Provide support and insights to ensure the best outcomes for difficult cases.
  • Provide participants with information, referrals, and follow-ups to ensure access to appropriate services and resources. Prioritize connections to community resources with a strong focus on securing stable housing.
  • Monitor and address concerns for shelter units, addressing any site safety concerns promptly.
  • Become familiar with the "California Tenant Laws" and how they relate to the HUD Permanent Housing Program.
  • Complete required annual training to stay current on best practices, program requirements, and professional development, ensuring the highest level of service and support for residents.
  • Think and act quickly and efficiently in emergencies.
  • Will drive on Agency business as needed and to support residents going to and from appointments within the Fresno community.
  • Physical presence is required in the office for all scheduled shifts.

ESSENTIAL JOB REQUIREMENTS

  • Possess a Bachelor's degree in Counseling, Psychology, Sociology, or related field or; an Associates of Arts degree and a minimum of two years' experience in a residential setting or shelter program or; a high school diploma / GED and a minimum of two (2) years' experience in a residential setting or shelter program.
  • Ability to work effectively with program residents, who are diagnosed with chronic mental illness.
  • Ability to obtain certified training in emergency first aid procedures and CPR.
  • Ability to communicate effectively in written and spoken English.
  • Ability to operate Homeless Management Information System (HMIS).
  • Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication / meeting platforms.
  • Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  • Ability to pass a pre-employment criminal background check.
  • Ability to pass a pre-employment drug screen and TB test.
