JOB SUMMARY

The Case Worker I, reporting directly to the Program Director, plays a vital role in providing comprehensive case management services to residents at our program locations. They will support residents in their daily activities and help them make meaningful progress toward achieving their short-term and long-term goals. By offering guidance, resources, and personalized support, the Case Worker I will contribute to the stabilization and success of residents, empowering them to build a strong foundation for independence and long-term well-being.

ESSENTIAL JOB RESPONSIBILITIES

  • Complete resident screenings, service plans and support the resident in achieving identified goals.
  • Provide residents with internal support and linkages to resources, as related to specific supportive needs.
  • Ensure residents fully understand and adhere to program rules and procedures by clearly explaining them upon intake and as needed. Provide each resident with a copy for reference.
  • Maintain thorough and accurate documentation, including specialized incident reports, crisis interventions, medication changes, and any notable changes in behavior. Ensure all client files and HMIS profiles are updated promptly and consistently to reflect the most current and accurate information.
  • Maintain positive relationships with all referral sources and community partners.
  • Collaborate with team members to effectively coordinate resident activities and foster social engagement. This includes leading and facilitating life skills groups for clients to support their personal growth, skill development, and empowerment.
  • Ensure the safety and well-being of all residents by closely monitoring the premises, with particular attention to properties where children are present. Take proactive steps to maintain a safe and secure environment for all residents, ensuring that both adults and children are protected at all times.
  • Serve as a resource for case staffing by collaborating with internal team members and participating in external conferencing to address complex decision-making challenges. Provide support and insights to ensure the best outcomes for difficult cases.
  • Conduct home visits in residents' units to ensure that the living spaces are safe, clean, and conducive to a stable living environment. During these visits, assess the condition of the unit and address any safety or maintenance concerns.
  • Work cooperatively with MAP (Multi-Agency Program) and Outreach / Navigator teams.
  • Observe and address correction of assigned household duties including all safety issues.
  • Become familiar with the "California Tenant Laws" and how they relate to the HUD Permanent Housing Program. Become familiar with how to enroll a resident which includes the rental agreement, HUD's 30% rent calculation, the program participation agreement and all other HUD related documentation including entry in the HMIS data system.
  • Complete a minimum of 40 hours of annual training to stay current on best practices, program requirements, and professional development, ensuring the highest level of service and support for residents.
  • Will think and act quickly and efficiently in emergencies.
  • Will drive on Agency business.
  • Physical presence at the program is required.

ESSENTIAL JOB REQUIREMENTS

  • 4-year bachelor's degree, OR 2 year associates degree, OR 2 years of experience in the human services related field.
  • Experience in a Mental Health residential setting, with an emphasis on supportive housing services preferred.
  • Ability to work effectively with program residents, who are diagnosed with chronic mental illness.
  • Current certified training in emergency first aid procedures and CPR preferred.
  • Intermediate computer skills in Office 365, Teams, Zoom and other communication / meeting platforms.
  • Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  • Ability to pass a criminal background check.
  • Ability to pass a pre-employment drug screen, and TB test.
