Tucker House Nursing and Rehab Center is seeking a Social Worker to join our team.

The Social Worker provides medically related social services, has an assigned caseload, and assists the residents in attaining or maintaining their highest practicable physical, mental and psycho-social well-being. The services provided are required to meet the professional standards of the social work practice and is consistent with both state and federal laws and regulations. The Social Worker guides the facility staff in matters of resident advocacy and protection while promoting residents' rights.

Certification: Current Social Work license in Pennsylvania preferred

Education: Bachelors, Masters, or Doctorate degree from an accredited Social Work or Social Welfare Program/School.

Experience: One year supervised social work experience working directly with individuals in a health care setting.
