POSITION TITLE : Case Management Supervisor, Coordinated System of Care (AB 2083) (Foster Care) & Specialized Caseloads
SALARY RANGE : $3,539.34 – $5,229.23 Biweekly; $92,022.84 - $ 135,959.98 Annual
REPORTS TO : Associate Director of Client Services (TBD)
LOCATION : May report to either the San Leandro or Concord offices.
The Regional Center of the East Bay (RCEB) is one of 21 Regional Centers that are community-based, private non-profit corporations under contract with the California Department of Developmental Services. RCEB provides intake, assessment, diagnosis, and coordinates community-based services for over 25,000 children and adults with developmental disabilities in Alameda and Contra Costa counties.

SUMMARY OF THE POSITION :
This position was established as a permanent position enacted by Assembly Bill (AB) 2083 adding legislative intent to develop a coordinated, timely, and trauma-informed system-of-care approach to serving foster children and youth who have experienced severe trauma. AB 2083, known as the Children and Youth System of Care, requires each county to develop and implement a memorandum of understanding (MOU), setting forth the roles of regional centers and other entities that serve children and youth in foster care who have experienced severe trauma by addressing systemic barriers to the traditional provision of interagency services.
AB 2083 requires the establishment of a state level Joint Interagency Resolution Team consisting of representatives from the Department of Social Services (CDSS), Department of Health Care Services, Department of Education and the Department of Developmental Services. As specified in W&I Code sections 16521.6(b)(1)(B)(i) and (ii), the Joint Interagency Resolution Team is required to ensure that a process is developed and implemented for counties and local partners that are parties to the MOU to request and receive interdepartmental technical assistance regarding the development of the MOU and to identify and secure the appropriate level of services.
Children and Youth System of Care Coordinator will have an important role in coordinating with local partners and securing appropriate level of services for children and youth in foster care.
In addition, this position will supervise a small unit of case managers responsible for a Specialized Caseload serving individuals with complex needs, in accordance with Welfare & Institutions Code (WIC) § 4640.6. The Supervisor will perform a full range of administrative and supervisory functions to ensure the delivery of high-quality case management services. Responsibilities include oversight of case documentation, monitoring consumer contacts, and ensuring compliance with regulatory requirements. This position will oversee case managers, including one for Porterville Developmental Center (PDC), and three specialized complex-needs caseloads, each with a 1 : 25 case manager-to-consumer ratio, focused on crisis stabilization, primarily serving adults, with a mix of children and youth as needed.
This role also involves facilitating and overseeing the development and implementation of Individual Program Plans (IPPs) for assigned consumers, evaluating the effectiveness of services provided, and stepping in to support case management functions as needed.
A key focus of this position is supervising case managers whose primary responsibility is to monitor and assist consumers transitioning from Institutions for Mental Disease (IMDs), Stabilization Training Assistance Reintegration (STAR) facilities, and / or correctional settings. The Supervisor will participate in coordination meetings with the Department of Developmental Services (DDS) to provide updates on these transitions and will ensure appropriate supports are in place to maintain the health and safety of each consumer throughout the process.
